Required Notice Under Texas HB 4224 (Health & Safety Code, Section 181.105)

Elite Wellness Chiropractic is committed to transparency and protecting your health information. In accordance with Texas House Bill 4224 and Section 181.105 of the Texas Health and Safety Code, we provide the following instructions to help you exercise your rights as a patient.

1. How to Request Your Medical Records

You have the right to request a copy of your health care records maintained by Elite Wellness Chiropractic. To request your records, follow these steps:

The form will ask for your name, date of birth, contact information, a description of the records you need, your preferred delivery method, and your signature authorizing the release.

Step 2: If you prefer not to use the online form, you may also submit a written request by email, phone, mail, or in person:

  • In Person: Visit our office during business hours (Monday–Thursday, 8:00 AM – 12:00 PM and 2:30 PM – 6:00 PM)

Please include your full legal name, date of birth, a description of the records you are requesting, and your preferred delivery method. A valid photo ID may be required for identity verification.

Step 3: Our team will process your request within 15 business days, as required by Texas law. You may be charged a reasonable fee for record copies, as permitted by state law.

2. How to Contact Our Licensing Authority

Elite Wellness Chiropractic and Dr. Mikala Booher, DC, are licensed by the Texas Board of Chiropractic Examiners (TBCE). You may contact the TBCE for any questions regarding licensure, professional standards, or disciplinary matters.

3. How to File a Consumer Complaint

If you believe your protected health information has been handled improperly, or if you have concerns about the care you received, you have the right to file a complaint. You may file a complaint with either or both of the following agencies:

File a Complaint with the Texas Board of Chiropractic Examiners (TBCE)

Step 1: Obtain the official complaint form by visiting https://tbce.texas.gov/complaint-information.html or by calling (512) 305-6708.

Step 2: Complete the complaint form in writing. Include your name, contact information, the name of the provider, and a detailed description of your concern.

Step 3: Submit the completed form by one of the following methods:

Step 4: The TBCE will acknowledge receipt of your complaint and conduct an investigation as appropriate.

File a Complaint with the Texas Attorney General's Office

If your complaint involves a potential violation of your health information privacy rights under Texas Health and Safety Code Chapter 181, you may also contact the Texas Attorney General:
